Our Commitment to You

At Liberty Creek Hospice, we provide only the best possible palliative care and hospice service from our most qualified, highly-skilled staff.

Hospice care is not about submission renunciation or giving up. Our philosophy is to provide uncompromising comfort through compassionate care. We want to help you live as well as you can, for as long as you can.

Therefore, if you want us to, we are here to:

  • Answer any questions that you may have and provide you with the information you need.

  • Help guide you with what lies ahead while allowing you to make decisions that are comfortable for you and your needs.

  • Keep a record of your decisions and ensure that we will do our best to make sure your wishes are fulfilled by those who will offer you the care and support you need and deserve.

  • Offer our unconditional desire to make sure that you are as comfortable as possible and you receive all the emotional and spiritual guidance for your family and you as you reach your remaining days.

  • To not only support you, but the people who are important to you as you approach the end of your life with dignity and comfort and during their bereavement.

Hospice Care: Where and When it's Needed

Hospice services aren't always by one's own facility. It is a service that comes to those who are terminally ill and are in need of comfort and care, whether one is in a hospital, a skilled nursing facility, a group home or a residential home.

Our mission is to provide you with uncompromising comfort through compassionate care, no matter where you are.

Our staff is specially trained to meet your needs in hospice services and to help manage your symptoms. We provide medications, pain management, medical equipment and supplies to meet your needs and to also aid you and your family with any emotional and spiritual guidance needed.

A patient's pain and symptoms, and by virtue of a physician's recommendation is deemed appropriate for palliative care and hospice service if any of these medical conditions exist:

  • Unremitting, progressive or chronic decline in health.

  • Increased or intolerable pain

  • Repeated confinements and hospitalizations

  • Recurrent or multiple infections

  • Profound weakness and incessant fatigue

  • Shortness of breath with or without oxygen support

  • Decreased ability to perform daily activities

  • Profound changes in mental conditions

  • Exhaustion from the patients and family members

  • Decision or desire to discontinue aggressive treatment

Being With Those You Love

Knowing what's to come and being able to prepare for it can help you get the support and care that you need.

One of our greatest fears is dying alone in a hospital, hooked up to tubes where your loved ones cannot even be with you. We also fear dying in pain. If possible, we would prefer to spend our final days pain-free, surrounded by those we love and in a setting that we are familiar and comfortable with.

"Hospice isn't the end. It's really joining two things--hope with service. Hospice never takes away

a person's hope and gives many services to achieve this."

The thought of death can be unpleasant and worrisome. Together with you and those you care about, we will be with you every step of the way. We are here to give you comfort and answer any uneasy questions, concerns and expectations as you go through this difficult time.

Liberty Creek Hospice's philosophy is that every patient has a right to a dignified death -- free from pain; and that your loved ones have the support they need when the time comes.
